What is the difference between HMR and RMMR?

HMR is for eligible people living in the community. RMMR is for eligible people living in approved residential aged care facilities.

Do I need a referral?

These programs require a referral from an eligible medical practitioner. A general enquiry is a first step, but does not replace a clinical referral.

What should I have ready for a home review?

Have the medicines you use available, including non-prescription products and supplements, along with any questions you want to discuss.

How do I check eligibility and costs?

Eligibility and funding depend on the program requirements and your circumstances. Ask your referring practitioner or contact the service before arranging a review.
